Insure Oklahoma is an innovative program Oklahoma has created to bridge the gap in the health care coverage for low-income

working adults. Under the Employer-Sponsored Insurance (ESI) program, premium costs are reduced for employees to 15%.

Employee Qualifications

 

 

To qualify, an employee should:

 

• Be between the ages of 19 and 64.

• Be an Oklahoma resident and meet citizenship

   guidelines.

• Have an annual household income at or below

   the income guidelines.

* Coverage for spouses and dependents may be

   available through Insure Oklahoma

 

*To qualify, the applicant's household must have a modified adjusted household income (MAGI) at or below the guidelines noted above.  Visit www.insureoklahoma.org for more information on how MAGI is determined.  Dependent children may count toward household size and may be eligible for Insure Oklahoma.

How is Household Income Determined?

What will I need to apply for Insure Oklahoma?  If you or your children have had or applied for SoonerCare in the past, you will have a username and password for the Oklahoma Health Care Authority webpage already.  If you do not know what that password is, you can go to www.insureoklahoma.org and select forgot password, or call 888-365-3742 and ask them what your username and password are?

How do I apply for Insure Oklahoma? You may submit the Insure Oklahoma Employee Application to your Employer.

You must receive an EEN number from your employer before you can apply for Insure Oklahoma Employer Sponsored Program.  

Once you have your EEN number, you can create and account at www.insureoklahoma.org and apply online.  You can also call 888-365-3742 and apply over the phone.

What if my children qualify for SoonerCare already? If your children qualify for SoonerCare, more than likely, you will qualify for Insure Oklahoma, however your children are not eligible.  If your children do not qualify for SoonerCare, it does not necessarily mean you and/or your children will not qualify for Insure Oklahoma. Click here to find out the difference between the income guidelines for SoonerCare and Insure Oklahoma Employer Sponsored Plan.
